Of course, doing so means entering the less familiar world of policy and politics. While many nurses do not initially feel prepared to operate in this space effectively, the reward is the opportunity to shape and influence future health policy. Regardless of political affiliation, individuals often grow concerned when considering perceived competing interests of government and their impact on topics of interest to them. The realm of healthcare is no different. Some people feel that local, state, and federal policies and legislation can be either helped or hindered by interests other than the benefit to society.

Consider for example that the number one job of a legislator is to be reelected. Cost can be measured in votes as well as dollars. Thus, it is important to consider the legislator’s perspective on either promoting or not promoting a certain initiative in the political landscape.

Obamacare was an Affordable Care Act (ACA) that was formed in 2010 to provide economical health care to over 30 million Americans. The purpose was to decrease healthcare cost for majority Americans so that they can have healthcare due to poor health of many Americans that did not have health insurance.

This was a summons for each individual to purchase. Also, individuals who had preexisting conditions could not be discriminated against from healthcare insurance companies. Insurance premiums was based on the amount Americans made as it relates to their salary. However, young adults paid higher premiums than older adults although they didn’t use their insurance as much due to having better health and etc.

Health insurance companies was not pleased due to the fact that they could not terminate anyone’s policy because they are in the business of making a profit and don’t want to come out of pocket too much. With that being said, all Americans were able to see their healthcare providers for preexisting conditions, check-ups, as well as signs and symptoms that could possibly lead to a more serious issue. There were millions of uninsured Americans that were going to hospital emergency rooms due to waiting under the wire for simple to chronic issues. The emergency room was their primary care physician! Its very expensive to visit the emergency room nowadays, the cost of the physician and being in the emergency are separate charges. The average emergency room visit is $1,389 which is up 176% since 2017 (Healthcare Cost Institute 2019).

All Americans should have access to affordable healthcare and no one should suffer due to their financial circumstances. The goal was to get at least 95% of employed Americans insured for healthcare which individual could get insured through the healthcare market place or through their job.

The numbers of insured individuals increased over time due to having penalties. According to Objective Analysis Effective Solutions, “Firms will increase their offer rates in response to employee preferences, which will change in response to the individual mandate. Specifically, workers will have greater demand for insurance, since there is a penalty associated with being uninsured.” (RAND Corporations).

Republicans failed the American people and could not get enough votes to pass legislation that would repeal and replace the ACA (Meyer 2017). Trump and his administration attempted to repeal Affordable Care Act as it relates to Obamacare. His proposal consists of, to make insurance fully tax deductible, health insurance to be sold across state lines, increase price transparency, and block Medicaid grants. The cost benefit analysis is a very messy world in politics.

The current republican party insist on proposing a bill that will eventually cut Medicaid which is a program to assist individuals that live in poverty. In the fine lines of this proposal, the healthcare plan will only provide tax cuts that will only benefit the rich. However, the primary goal of legislators is to be re-elected and as it relates to the cost benefit analysis, its in their efforts to repeal or replace Patient and ACA. There are some legislators that would like to keep things the way they are, just with slight improvement and then you have others that want to totally changed and/or replaced (Milstead and Short, 2019). If there were to repealed and not replace the ACA, many of insured Americans will immensely backout as well as premiums and out of pocket expenses will increase.

Due to this, will cost many to not have health insurance and would impact society in an unfavorable way, especially for those individuals that live in poverty and rural areas. Which will lead to many Americans going untreated and mortality rates increasing. Situations as such, can either affect legislators in a positive or negative way when it comes to being reelected and getting the support from the political party.

Politics and the Patient Protection and Affordable Care Act

The Affordable Care Act was signed into law in 2010, by President Barack Obama, which addressed several issues including health insurance coverage, preventative care, coverage for preexisting conditions, and healthcare costs (Rand Health Care, 2019). The purpose of the ACA was to reduce the number of uninsured citizens, make health insurance more affordable, and expand access to health care, meaning that millions of Americans will receive the health insurance coverage and access for healthcare services that they need (ACA, 2019). Prior to the ACA, healthcare costs were rising in America and there were millions of Americans denied insurance coverage which had an impact on society. This was a top concern to the Obama administration because people who needed medical insurance were either denied or cancelled. Many people were not seen under a primary care physician, had no way to pay out-of-pocket for medical expenses, and some were not seeking medical attention at all.

People who seek medical treatment while uninsured has caused health care providers in hospitals to be overwhelmed from providing uncompensated care. However, health care is a concern for many. And depending on the political party, the decisions that are made by legislators whether to repeal/replace the ACA may have an impact on society as they are obligated to do what is best for them to be reelected versus what matters.

In contrast, I think that the cost-benefit analysis in terms of legislators being re-elected in efforts to repeal/replace the ACA may be a difficult decision for legislators because they want to remain in office. However, to find the support to continue to be reelected, legislators must be aware of how certain votes will affect their ability to stay in office. They must consider the needs of the community and the desire of their political party.

Because although it is important to them to become reelected, I think that it is also important to them that they make a decision that is based on the political agenda versus what matters to society. Although, some are in favor of not making any changes to the ACA, there are some that are willing to repeal or replace it (Milstead and Short, 2019). If the ACA were repealed and not replaced, American people would be highly affected with no insurance coverage and insurance companies can again deny or delay care to those who are suffering with chronic or preexisting conditions.

If repealed, it is estimated that over 20 million Americans will lose their healthcare coverage (National Conference of State Legislators, 2011). It will also affect jobs, cause a drop in the number of insured Americans, increase out-of-pocket-costs, decrease the quality of care, and increase the mortality rate.

Ideally, the primary goal of legislators is to be re-elected and to increase their chances for political support. The decisions that are made by legislative leaders in recommending or positioning national policies are most likely affected by their political party’s agenda.  Their decision as opposed to being reelected should be solely focused on what is right for the people versus what is right for the party.

With that being said, who may benefit the most is the policy makers because “ policy makers are not necessarily focused on how real people will be affected by the change….but rather on how the change will affect their own reelection chances” (Mildstead & Short, 2019).
