Nursing Care Plan Writing

Nursing Care Plan Writing

Most nursing students can agree that writing care plans is not one of their favorite things to do. However, nursing care plan writing is a must for every student looking to succeed in the nursing field. Care plans are mandatory in every class in the nursing career including mental health, community care, and intensive care patients. Writing a nursing care plan means staying up almost all night and creating time to carry out massive research to come up with a piece that will please the tutor. If you want to achieve a high grade, then you should also ensure that you not only finish the assignment on time but also write a high-quality care plan.

The main goal of a nursing care plan is to provide a clear direction on the personalized health care to be provided to a patient. Nursing care plan writing depends on the diagnosis of a patient and their individual needs to determine their treatment and recovery process.  A care plan is a form of communication between two or more nurses regarding the wellbeing of a patient. Every nurse is required to update the care plan each time they visit or attend to the patient. In the middle of rounds or after every shift, one is required to hand over the nursing care plan to the nursing staff.

Why you Should Seek Professional Help in Nursing Care Plan Writing

Nursing care plan writing helps in the documentation. They help nurses in determining the type of action and observation they need to make and the instructions to give to the particular patient and their loved ones. Nursing care plans also help in assigning the perfect nurse to a patient. In some cases, a patient may need the specialized care of a specific nurse with skills that fit the needs of the patient. Using a nursing care plan helps to identify the nurse who matches the requirements of the patient in question.

As we can see above, a nursing care plan is very critical writing and requires a lot of concentration and accuracy. It also requires a nurse to dedicate their time in writing it in order to avoid including false information. Failing to write a nursing care plan keenly may affect the well-being of a patient and may eventually jeopardize the career of the nurse.

For this reason, it is important to seek professional help if you feel you are not in the position to write an accurate nursing care plan. This type of writing puts excess pressure on nursing students, especially those with other priorities to take care of such as family and side-jobs. Nursing care plan writing helps the tutor find out whether you are ready to move on to the next step and practice nursing in real life. You do not have to push yourself so hard if you are unable to write a nursing care plan on your own. You should know that there are other students just like you who seek expert help in their nursing care writing.

Example Nursing Care Plan Form

Student Name   Date 11/06/2022
Patient (initials only) XXX Patient Medical Diagnosis Myocardial Infarction
Nursing Diagnosis (use PES/PE format)  Acute pain related to myocardial ischemia as evidenced by the patient’s history of non-radiating chest pain

Assessment Data

(subjective and/or objective pieces of data that lead to the nursing diagnosis)

Goals & Outcome

(Two statements are required for each nursing diagnosis.  Must be Patient and/or family focused; measurable; time-specific; and reasonable.)

Nursing Interventions

(List at least three nursing or collaborative interventions with rationale for each goal & outcome.)

Rationale

(Provide reason why intervention is indicated / therapeutic; provide references.)

Outcome Evaluation & Re-planning

(Was goal met?  How would you revise the plan of care according the patient’s response to current plan?)

1.     Non-radiating chest pain (7/10 severity)

Statement #1: Within the first 15 minutes of the patient’s arrival at the emergency department, the nurse will administer drugs to ease the chest pain

Statement #2. The patient will practice relaxation techniques to reduce anxiety and improve healing

1.Administer 6 L/minute of oxygen via a face mask to maintain oxygen saturation above 90%.

2. Give sublingual nitroglycerine 0.4 mg five minutes apart up to three times. Educate the patient to dissolve the drug under the tongue, and not rinse the mouth or spit for 5 minutes after the administration

3. Instruct the patient to do relaxation techniques such as deep and slow breathing, visualization, distracted behavior and guided imagery. Assist as needed

1.Blood flow to the myocardial tissues is compromised in myocardial infarction owing to a partial or complete occlusion of the coronary artery. Oxygen therapy improves oxygen supply to the ischemic myocardium, reducing infarct size and consequent consequences (Li et al., 2018). The patient’s improved breathing and SPO2 will indicate whether oxygen should be withheld, stopped, or continued.

2. Sublingual nitroglycerine provides a symptomatic relief. Nitroglycerine is an organic nitrate that produces systemic vasodilation, lowering both preload and afterload and decreasing myocardial oxygen demand (Bibli et al., 2019). Nitroglycerine also improves coronary collateral circulation, lowers blood pressure, and increases the heart rate (Bibli et al., 2019). Consequently, the amount of work the heart has to do lessens, lowering the chest pain.

3. Patients with myocardial infarctions are concerned about their imminent mortality as well as the irreversible change in their health and socioeconomic situation. In addition to chest pain and dyspnea, they may exhibit anxiety symptoms such as restlessness and diaphoresis, as evidenced in the 50-year-old patient. Anxiety may be reduced by using relaxation techniques such as deep and slow breathing. Relaxation techniques also help reduce pain perception and response while providing a feeling of control over the situation

The goal was accomplished. After 24 hours, the patient’s oxygen saturation improved and he was completely weaned off oxygen.

The goal was accomplished. Sublingual nitroglycerine was given, and the patient’s pain level dropped from 7/10 to 3/10 after 20 minutes.

3. The objective was met. The patient demonstrated efficient breathing techniques, and his anxiety decreased significantly.
