While this is a diverse set of advanced physical assessment skills where do you place your greatest level of confidence and what areas need more development.  Of these areas of advanced physical assessment where to you struggle in your nursing practice most when providing patient-centered education?

Sample Discussion 6 Approach

This week’s readings cannot be more dissimilar. While I cannot say that I am an expert on either topic, I do sit on my facility’s mental health environment of care checklist (MHEOCC) committee. This committee’s primary goal is to assess the inpatient mental health unit’s environment to identify ligature and suicide prevention safety issues (U.S. Department of Veteran Affairs, 2018). Essentially the team puts themselves in the suicidal Veteran’s mindset to identify hazards that could cause them and staff harm. This process has reduced the suicide rate by 2.64 per 100,000 to 0.87 per 100,000 admissions (U.S. Department of Veteran Affairs, 2018). I encourage you to utilize this checklist in your facilities.

In addition to these concepts, I have learned about the use of the Columbia-suicide severity risk scale (C-SSRS) (Substance Abuse and Mental Health Services Administration [SAMHSA], 2014). Suicide is the second leading cause of death for those 10 to 25 years old (Erbacher & Singer, 2018). The use of a validated tool such as the C-SSRS provides a standard practice to assess a patient’s intent to commit suicide (Erbacher & Singer, 2018). Warning signs for risk include ideation, purpose, and a plan (Erbacher & Singer, 2018). At my facility, we use this tool to trigger further intervention and implement a patient focus safety plan.

In medical practice, screening questions include depression, anxiety, substance abuse and personality disorders (Bickley & Szilagyi, 2017a). While knowledge of assessment processes for suicide and the mental health environment is essential; I would not rank myself as adept at mental health patient-centered education.

A female patient’s health history includes menarche, menstruation, and menopause (Bickley & Szilagyi, 2017b). Information about pregnancy, vulvovaginal symptoms, sexual health, pelvic pain, and sexually transmitted diseases continue to be essential for assessing female well-ing (Bickley & Szilagyi, 2017b). Necessary to determine patients ages 20 to 24 years for sexually transmitted infections such as chlamydia (Bickley & Szilagyi, 2017b). Monitoring for depression in women of reproductive age key after childbirth and neonatal loss (Centers for Disease Control and Prevention [CDC], 2020). Clinical factors, including a family history of inherited medical conditions and gene disorders, are critical components of assessing female reproductive health (American College of Obstetricians and Gynecologists [ACOG], 2020).
