This week you have studied cardiovascular, peripheral vascular, and respiratory advanced physical assessment. What additional resources for these advanced health assessment skills have you found beneficial in developing your knowledge and psychomotor skills this week?

Example Discussion 2 Approach

Assessments of the cardiovascular, peripheral vascular, and respiratory systems are especially important as each system can have a significant impact on the other resulting in acute and chronic health concerns. Updated guidelines in regards to cardiovascular health focus’s more on lifestyle changes of the patient to manage and control health risks instead of simply prescribing medications and other treatments(Lefler et al., 2016).  Resources that I started using over the weekend with thanks to my fellow students sharing in their discussions is “thePoint” which provides an assessment checklist to cover for both physical and questions to ask the patient and is available in the front of the Bates textbook. This also provided the ability to listen to heart and lung sounds along with questions you can answer. The other resource that has been extremely helpful to me is on You-tube and is called RegisteredNurseRN. This site provides a complete breakdown for anatomical locations, normal and abnormal sounds along with the rationale for each part. For those of us who have not been completing many physical assessments in the last few years, it is a great refresher. What I also appreciated was the quizzes that were available to listen to the sounds and then identify what they were. 

Shadow Health is still a work in progress for me in regards to the organization and ensuring that I am asking for a full history. When I completed the assessment on Tina in regards to the wound on her foot, I made the mistake of making it a focused assessment instead of continuing with an overall complete assessment which had a significant negative impact on the questions I asked. But each time I complete an assessment I feel like I am learning and understanding more which will continue to improve my ability. 

I’m curious if anyone is submitting their shadow health assessments? I did not anywhere if we are supposed to submit them or if they are our own practice tools and we only need to submit our practice experience form at the end. Clinical presentation of left-sided heart failure depends on the stage of heart failure the patient is at. A four-stage process is used by the American Heart Association (AHA) and the American College of Cardiology (ACC) and is based on risk, damage to the heart, and clinical symptoms. With the first stage, a patient has risk factors such as hypertension or other cardiovascular diseases. The second stage can progress slowly or quickly depending on if the patient is active in taking their medications and maintaining a healthy lifestyle. Clinical presentation is shortness of breath(SOB) with activities, SOB when lying down, paroxysmal nocturnal wheezing, a cough, and sometimes wheezing (Bickley et al., 2016). Other presentations can be fatigue and weakness along with pedal edema that will increase in the third stage along with clinical findings of crackles with lung auscultation, tachycardia, S3 gallop, and increased jugular venous pressure (Patterson & Felicilda-Reynaldo, 2018).
