Intervention in public health is any effort or policy that attempts to improve mental and physical health at a population level. Public health interventions may be run by a variety of organizations, including government health departments and non-governmental organizations; in medicine, it’s a treatment, procedure, or other actions taken to prevent disease, or improve the health of a community, carried out by stakeholders, nurses, community social workers, donors.

An example of a need currently being addressed by an intervention in patients with diabetes mellitus already in place for the community for an outpatient clinic appointment, a community group for patients with chronic disease, especially for patients that have been discharged to home after admission to the hospital. The intervention to this targeted population will include follow up programs through visits by public health nurses, or patients in the clinic visits, or by telephone; the overall aim is geared to stress the need of adhering to diabetes self-care strategies, with an impact to improve the quality of care for that patient and the overall community.
