For this assessment, you will create a 4 page report on an interview you have conducted with a health care professional. You will identify an issue from the interview that could be improved with an interdisciplinary approach, and review best practices and evidence to address the issue.
As a baccalaureate-prepared nurse, your participation and leadership in interdisciplinary teams will be vital to the health outcomes for your patients and organization. One way to approach designing an improvement project is to use the Plan-Do-Study-Act (PDSA) cycle. The Institute for Healthcare Improvement describes it thus:

The Plan-Do-Study-Act (PDSA) cycle is shorthand for testing a change in the real work setting—by planning it, trying it, observing the results, and acting on what is learned. This is the scientific method adapted for action-oriented learning…Essentially, the PDSA cycle helps you test out change ideas on a smaller scale before evaluating the results and making adjustments before potentially launching into a somewhat larger scale project (n.d.).
You might also recognize that the PDSA cycle resembles the nursing process. The benefit of gaining experience with this model of project design is that it provides nurses with an opportunity to ideate and lead improvements. For this assessment, you will not be implementing all of the PDSA cycle. Instead, you are being asked to interview a health care professional of your choice to determine what kind of interdisciplinary problem he or she is experiencing or has experienced in the workplace. This assessment will introduce the Plan-Do-Study-Act (PDSA) Model to create change in an organization. By interviewing a colleague of your choice, you will begin gathering information about an interprofessional collaboration problem that your colleague is experiencing or has experienced. You will identify a change theory and leadership strategies to help solve this problem.
NURS-FPX4010 Assessments 2 Scenario
This assessment is the first of three related assessments in which you will gather interview information (Assessment 2); design a proposal for interdisciplinary problem-solving, (Assessment 3); and report on how an interdisciplinary improvement plan could be implemented in a place of practice (Assessment 4). At the end of the course, your interviewee will have a proposal plan based on the PDSA cycle that he or she could present to stakeholders to address an interdisciplinary problem in the workplace.
For this assessment, you will need to interview a health care professional such as a fellow learner, nursing colleague, administrator, business partner, or another appropriate person who could provide you with sufficient information regarding an organizational problem that he or she is experiencing or has experienced, or an area where they are seeking improvements. For this assessment, you will report on the information that you collected in your interview, analyzing the interview data and identifying a past or current issue that would benefit from an interdisciplinary approach. This could be an issue that has not been addressed by an interdisciplinary approach or one that could benefit from improvements related to the interdisciplinary approach currently being used. You will discuss the interview strategy that you used to collect information. Your interview strategy should be supported by citations from the literature.
Additionally, you will start laying the foundation for your Interdisciplinary Plan Proposal (Assessment 3) by researching potential change theories, leadership strategies, and collaboration approaches that could be relevant to issue you have identified. Please be certain to review the scoring guide to confirm specific required elements of this assessment. Note that there are differences between basic, proficient and distinguished scores.

NURS-FPX4010 Assessment 2: Interview and Interdisciplinary Issue Identification Sample
John Davis works as a Registered Nurse at the Abbott Northwestern Hospital. His tenure there now exceeds seven years. After three years of experience in the emergency department, he was reassigned to the Trauma Surgical Intensive Care Unit. He is a certified trauma nurse with a Bachelor of Science in Nursing and completed the Trauma Nursing Core Course (TNCC). Although he thinks the institution provides excellent care to its patients, he believes it places too little emphasis on the well-being and satisfaction of its employees, particularly its nurses.
He has TNCC accreditation and assists patients using balloon pumps, renal replacement treatment, extracorporeal membrane oxygenation devices, and roto-prone beds. However, he has not been promoted despite working in a highly specialized field of nursing care for a long time. He also noted that upper-level management’s unfamiliarity with floor nursing prevented it from making informed decisions about the unit’s operations. Not only does he feel that primary nursing care is being neglected, but he has also witnessed procedures like blood glucose monitoring, central line care, and quality control checks being ignored.
He reports that it is not uncommon for him to arrive at work, perform his morning assessments, and realize that the central line of the patients has been due for dressing for more than two days. Assessing the patient’s blood glucose levels takes an additional 15 minutes because of the quality control check on the monitor. Many basic supplies, such as sterile dressing, syringes, needles, and lancets, are not stuffed in rooms. He claims he has raised similar concerns repeatedly with the administration in unit meetings without success.
John thinks there is room for improvement in terms of interdisciplinary cooperation in the organization. According to him, everyone from the doctors, nurses, therapists, and essential personnel like nursing assistants and maintenance work together collaboratively. However, those in higher positions, including charge nurses and the unit supervisor, are not aware of the flow of activities in the unit. They blame the team when their directives are not followed or their expectations are not met. He believes that if senior management had a better understanding of how it felt to work on the floor, more radical ideas would be explored, and more obstacles would be resolved.
Issue Identification
Neglecting essential nursing responsibilities like central line site care, monitoring patient blood glucose levels, ignoring quality control checks, and restocking essential items calls for an evidence-based interdisciplinary approach. The approach would be helpful if adopted in the organization because nurses, nurse assistants, and administrators can all participate in coming up with solutions to these identified issues.
Lewin’s Theory of Change for an Interdisciplinary Solution
Lewin’s theory of change may provide an interdisciplinary approach to fixing this problem. It has three distinct phases; unfreezing, transitioning, and unfreezing. During these phases, it will be essential for nurse managers, nurses, and nurse assistants to collaborate on finding strategies to solve the existing challenges. Within the unfreezing phase, a nurse leader will recognize the problems that need solutions (Weiss et al., 2019). Professionals in the nursing field will recognize the problem and inspire others to realize the requirement for a solution.
They can accomplish this by announcing at a unit gathering and explaining how failing to adequately care for the central lines puts patients at risk of life-threatening infections. They can describe how nurses’ responsibilities are negatively impacted by inadequate restocking of supplies and lack of quality control checks. Nurses may be unable to function properly during an emergency since essential supplies are unavailable in the rooms.
During the transitioning phase, nurse leaders develop an implementation strategy and mobilize their teams to try the new strategies (Weiss et al., 2019). For instance, if central line dressing changes must occur at regular intervals, the rooms could be split into two groups or more. The caregivers on the day shift can take care of one set of rooms, while the night staff can handle the other. This approach will guarantee that central line dressings are replaced at the prescribed intervals.
Additionally, before every shift, quality control inspections should be performed on blood glucose meters and rooms to be restocked; certified nursing assistants are responsible for these tasks. Rooms should have a predetermined quantity of all supplies; thus, a checklist should be used to ensure that everything is available. Mentoring and open communication are essential throughout this stage to keep the teams from losing track of the new and enhanced strategies.
The final phase is stabilization into the new routine and integrating the adjustments into the unit’s established procedures. Here, individuals from various fields collaborate to establish a brand-new standard (Weiss et al., 2019). At this point, nurse leaders in a leadership role should check in frequently to ensure the new procedures are followed. Both registered nurses and their assistants must adjust to the new standards of excellence.

Bergstedt and Wei (2020) point out that when effective leadership is employed in organizations, job satisfaction and worker engagement in work are improved. The leaders ensure that the quality of healthcare availed to the patient is high and that all workers are involved in decision-making. Nurse managers need to implement leadership techniques to increase employees’ commitment and tolerance for new changes. One strategy is to ensure that the organization has shared values and beliefs that align with those held by the employees (Bergstedt & Wei, 2020). Most healthcare workers choose to work in this realm because they believe their efforts can make a difference in the lives of their patients. They also believe that the healthcare sector helps to market the values that they own. Therefore, the organization must ensure that all the workers are allowed to express their values and expectations; this shows that the organization respects and accommodates all persons despite individual differences.
Similarly, the employment of open communication and the provision of feedback is another vital strategy. Nurse leaders should continuously engage in dialogue with the patients and healthcare workers, as it shows genuine concern for their welfare. Constructive feedback should be provided to the employees so they know what was not done well and what should be improved; this can motivate the workers to work harder to improve their skills (Bergstedt & Wei, 2020). Acknowledging and rewarding the efforts of team members is another way to boost productivity. Acknowledging team efforts and recognizing obstacles and triumphs are two ways to recognize the team’s efforts (Lee, 2020). The quality of care provided to patients and the workplace environment can be enhanced by collaborative interdisciplinary efforts. Conclusion
It is vital for workers in the healthcare sector to voice their concerns to the top management for consideration. The management must ensure that the workers’ grievances are looked into and that effective strategies are formulated to resolve the existing challenges. Employees should be given opportunities to contribute to decision-making as most new implementations affect them directly. There is a need to stress the importance of collaboration at the workplace; there is a strong correlation between teamwork and the quality of healthcare an organization provides. In addition to summarizing the key points of Assessments 2 and 3, you will provide stakeholders and/or leadership with an overview of project specifics as well as how success would be evaluated—you will essentially be presenting a discussion of the Plan, Do, and Study parts of the PDSA cycle. Again, you will not be expected to execute the project, so you will not have any results to study. However, by carefully examining the ways in which your plan could be carried out and evaluated, you will get some of the experience of the thinking required for PDSA.
When creating your PowerPoint for this assessment, it is important to keep in mind the target audience: your interviewee’s organizational leadership. The overall goal of this assessment is to create a presentation that your interviewee could potentially give in his or her organization.
