Healthcare has been a policy item and a topic of debate not only in recent times but as far back as the administration of the second U.S. president, John Adams. In 1798, Adams signed legislation requiring that 20 cents per month of a sailor’s paycheck be set aside for covering their medical bills. This represented the first major piece of U.S. healthcare legislation, and the topic of healthcare has been woven into presidential agendas and political debate ever since. As a healthcare professional, you may be called upon to provide expertise, guidance and/or opinions on healthcare matters as they are debated for inclusion into new policy. You may also be involved in planning new organizational policy and responses to changes in legislation. For all of these reasons you should be prepared to speak to national healthcare issues making the news.

In this Assignment, you will analyze recent presidential healthcare agendas. You also will prepare a fact sheet to communicate the importance of a healthcare issue and the impact on this issue of recent or proposed policy.

  • Review the agenda priorities of the current/sitting U.S. president and the two previous presidential administrations.
  • Select an issue related to healthcare that was addressed by each of the last three U.S. presidential administrations.
  • Reflect on the focus of their respective agendas, including the allocation of financial resources for addressing the healthcare issue you selected.
  • Consider how you would communicate the importance of a healthcare issue to a legislator/policymaker or a member of their staff for inclusion on an agenda.

Use the Agenda Comparison Grid Template found in the Learning Resources and complete the Part 1: Agenda Comparison Grid based on the current/sitting U.S. president and the two previous presidential administrations and their agendas related to the public health concern you selected. Be sure to address the following:

  • Identify and provide a brief description of the population health concern you selected and the factors that contribute to it.
  • Describe the administrative agenda focus related to the issue you selected.
  • Identify the allocations of financial and other resources that the current and two previous presidents dedicated to this issue.

You can use Microsoft Word or PowerPoint to create your Fact Sheet or Talking Point Brief. Be sure to address the following:

  • Summarize why this healthcare issue is important and should be included in the agenda for legislation.
  • Justify the role of the nurse in agenda setting for healthcare issues.

Presidential Agendas – Educational Loan Reform

According to Forbes 2018 article highlighting the student loan debt statistics, “Student loan debt is now the second highest consumer debt category – behind only mortgage debt – and higher than both credit cards and auto loans” (Friedman, 2018). Per the statistical data reviewed in this article, the United States is home to over 44 million student loan borrowers with a collective student loan debt of $1.5 trillion; This number continues to rise annually as the cost of higher education continues to inflate.

In an attempt to manage this mounting debt, while ensuring access to higher education and management of student loan programs, educational loan reform has made center stage on the agenda ticket for the Bush, Obama, and Trump Presidential Administrations. President Bush
In 2007, this administration signed legislation expanding the Pell Grant’s access and raising the loan borrowing limits of Stafford loans in an attempt to make college affordable to students, expanded federal loan forgiveness for Math, Science, and Special Education Teachers (2006), and enacted the College Cost Reduction and Access Act (2007). This Act amended the Higher Education Act of 1965, allowing a new Income-Based Repayment plan to help make repaying college debt more feasible.
The hallmark of President Bush’s educational initiative was the No Child Left Behind movement, which was focused on ensuring educational success from the early childhood years.

President Obama
Following in the footsteps of the previous administration, President Obama also aimed to keep college accessible for the average and the low-income American. Obama signed legislation in early 2010 eliminating fees paid to private banks, such as student loan lender Sallie Mae, allowing the savings to expand the Pell Grant an additional $68 billion over the next 11 years. The legislation also capped the repayment of student loan payments at 10% of income above a basic living allowance, known as the Administration’s “Pay as You Earn” plan.   

President Trump
After the Bush and Obama administration’s attempts to create positive change to America’s student loan crisis, President Trump’s Administration passed the Tax Cuts and Jobs Acts (2017) which allowed for loan forgiveness for permanently disabled and deceased student loan holders. In March of 2020, President Trump further proposed additional changes: First, consolidate the four income-driven payment (IDR) models into one to allow the Dept. of Education to better manage plans, and change the time frame of loan forgiveness of undergrad and graduate loan programs. Second, this proposal would eliminate the Public Service Loan Forgiveness (PSLF) program, rejecting partisan treatment of public service workers. When comparing the three Presidential Administrations highlighted above, President Trump’s proposed 2020 plan is noted to be the most controversial with a lengthy list of oppositional perspective. Critics of the proposed plan note that elimination of the PSLF would discourage individuals from entering the public service professions, which has the highest trajectory of growth in the job market (includes Nurses and Teachers); This plan also increases the number of years allotted for forgiveness of graduate student loans by an additional 5 years. Trump’s plan does not account for the increase of interest rates, cost of living increase, or the most recent job losses associated with the Covid-19 pandemic.

If given the option to rally for student loan reform, I would follow in the steps of Presidents Bush and Obama by focusing on tax incentives for students aspiring for higher education, maintain a low threshold on educational loan interest rates, and provide incentive to those choosing to seek employment in positions in highest demand of public service.
