Re: Topic 5 DQ 1

Nurse managers and nurse leaders have a role in reengineering healthcare. As an organization grows, nurse managers and nurses must grow with it. This is evident through the Synergy Model. This is a patient centered model that focuses on enhancing patient care by matching the qualities of the patient with the competencies of the nurse. The main focus of the nurse is to care for the patient. The eight characteristics that put together the Synergy Model are resiliency, vulnerability, stability, complexity, resource availability, participation in care, participation in decision-making, and predictability. In the Synergy Model, there are five levels of competency from competent to expert, in eight various categories. Nurse leaders are considered risk takers. Although they have always focused on care delivery, they must now focus on the outcomes of the delivery provided. In addition, during this reengineering of healthcare process, nurse leaders are stepping up and using their skills and expertise to offer new inventive strategies to engage in healthcare reform (Helbig, 2018).

Nurse managers and nurse leaders contribute to the reengineering of health care by being involved directly with patient outcomes and patient satisfaction. They have the responsibility to provide evidence-based practice and patient safety to all patients within the health care system. Nurse managers and nurse leaders have the opportunity to evolve healthcare by gathering data from patients and also by nursing research. Nurse managers and nurse leaders are health professionals that are dedicated to improving quality of care. According to Helbig, “It is through policy that nurses influence the quality of care delivered. Nurses impact health care organizations through patient outcomes and patient satisfaction. By influencing policy, nursing can shape how health care is provided in the future. Nurses play “a unique role in formulating policy” (Helbig, 2018). The synergy model is used to match the needs of the patient to the competences of the nurse. The model consists of resiliency, vulnerability, stability, complexity, resource availability, participation in care, participation in decision making, and predictability. Nurse managers and nurse leaders work by being the risk-takers in the system and providing innovation for growth. They do not necessarily provide direct patient care but work to ensure their staff is providing the best quality care for the patients.

Health care requirements need to adapt to changes based on the needs and demands of the community being served. There-fore reengineering is necessary to maintain safety in practice, improve processes, and increase efficiency in care delivery while saving costs. A manager plays an important role in advancing and keeping the healthcare system in progress. The nurse manager initiates and coordinates the health care team in delivering high-quality patient care services. For these managers or leaders to achieve this goal, several principles have to take place. These include ethical decision making and adhering to professional codes. Reengineering health care essay. “Ethical decision making like any other decision making requires the manager to gather information, identify the problem, generate alternatives, or select an alternative to implement and evaluate results. Ethical decision making depends on the identification of the ethical principles that guide the selection and expectations of the outcome” Professionally, nurse managers should keep up to date with legal aspects in the health care system in order to protect patients, staff, and the interests of the organization.

According to the textbook, nurse leaders have a major role in reengineering health care. They are responsible for putting the right people in place to ensure that quality improvement processes continue and that they increase efficiency and improve outcomes (Helbig, 2018). Nurse leaders are involved in the day to day workflows and can suggest, implement and evaluate change(s). They also are aware of those employees who can be influential, both positive and negative, and involve the correct people to help change(s) be successful. Nurses are the largest group of healthcare providers and are among the most trusted. They can impact health care organizations through patient outcomes and patient satisfaction (Helbig, 2018). Nurse leaders understand the importance of quality improvement toward patient outcomes and patient satisfaction and work hard with their team to make it happen.

Continuous quality improvement is used to expand clinical practice and is based on the conception that there is forever an opportunity for improvement. CQI is the responsibility of all nurses. It is crucial when addressing the challenges of today’s healthcare industry. When an issue is identified, it is brought to a committee that discusses quality improvements. Data is then collected and examined to find ways to make improvements. These improvements are then put into practice changes. Nurse leaders understand how important CQI is to make changes and improve patient outcomes (Helbig, 2018).

An example of how CQI has been applied in my hospital is with the use of indwelling foley catheters. A few years ago, my facility noticed that the UTI infection rate was very high for patients that have foley catheters. The hospital monitored this data and made several practice changes. Now, patients do not get foley catheter even if incontinent. There are only certain criteria for patients to get an indwelling catheter. In addition, patients are placed on top of an absorbent pad. Adult diapers are not used. Patients are assessed more often. They are clean and turned more often. Most importantly, due to these practice changes, the rate of UTIs have gone down tremendously.

Re: NRS 451 Topic 5 Discussion DQ 2

Continuous quality improvement is based on the identification of delivery improvement. This can be determined by patient outcomes, organization structure, or by the processes. In healthcare, there is always opportunity for improvement which is the definition of continuous quality improvement. Helbig explains, “Quality improvement teams are organized when data need to be collected and analyzed to find new and innovative ways to make improvements. The findings or quality improvement strategies are then translated into practice changes. It is important that management meets with staff and empowers staff to implement changes” (Helbig, 2018).NRS 451 Nursing Leadership and Management Discussion

An example of continuous quality improvement in my current practice would be infection prevention. My hospital has noticed an increase with infections associated with PICC lines. We as a unit gathered information about the increase of infections. We have implemented mandatory dressing changes upon admission or if the dressing was disrupted. We also do CHG wipes every shift for all patients with PICC lines. The rate of infection has decreased but we are still working to improve the rate by acquiring more data and implementing new strategies in patient care.
