A logic model is a systematic and visual representation for showing the relationship between program resources, activities, outputs, and outcomes the program hopes to achieve. The main components of a logic model are inputs (resources), activities (interventions or strategies), outputs (evidence of activities; products), and outcomes (results or effects for short-term, intermediate, and long-term outcomes).

This assignment requires you to support your position by referencing three to four scholarly resources.

LITERATURE EVALUATION TABLE:

Identify a barrier or issue in health care that you want to address through your capstone project paper. Perform a literature review on the barrier or issue using the GCU Library. Present 8-10 peer-reviewed primary research articles. These articles should be either qualitative or quantitative.

You may use either health care or nursing peer-reviewed journals; however, at least half of the primary articles must come from peer-reviewed nursing journals. The peer-reviewed articles must consist of research published within the last 5 years.

Use the “HLT-494 – Literature Evaluation Table” template to document your detailed assessment of each primary study. This document helps break down the components of each article to ensure the article meets the requirements for a professional nursing paper. Ensure that each of your articles provides information that meets each criterion listed. Assess the research and provide a detailed and clear explanation how each article supports your capstone project and evidence-based practice (between five and six sentences).

PART 1: CHANGE PROPOSAL TOPIC:

Identify the stakeholders needed to support the implementation of the proposed plan. Explain how these stakeholders are vital to implementing the change plan.

Summarize potential communication strategies to internal and external stakeholders to facilitate a change plan.

Discuss the impact this change plan will have on the organization and its stakeholders.

Identify expected outcomes and potential barriers to implementing the change plan.

Discuss steps for overcoming the potential barriers.

Create a timeline that visually details the implementation steps of the capstone project. The timeline should be separated by weeks. It is helpful to organize the implementation plan into smaller, short-term goals. The timeline should estimate when the following tasks should be completed.

Implementation Steps:

Identify the stakeholders needed to support the implementation of the proposed plan.

Identify communication strategies to internal and external stakeholders to facilitate a change plan.

Review your logic model, change proposal, and initiation plan. Describe in detail how the overall change plan will be evaluated, and the resources needed to evaluate the project. Discuss the evaluation process in relationship to the projected outcomes.

Create a dissemination plan. Explain how the outcome of the project will be disseminated externally (outside the setting to health care community) and internally (unit or hospital where the change process has taken place). A detailed plan answers the questions who, what, where, how, and when.
