Explanation & Answer length: 1500 Words

In the Phoenix VA Healthcare System, hemodialysis nurses enter patients’ vital signs on dialysis as free text in the medical record. This process causes the data to be missing from the vitals tracking system. The absence of vital signs in the tracking system causes incomplete vital signs and makes it impossible to track and trend the vital signs. An audit of 6 records showed an average of 12 vital signs per dialysis session was not captured correctly. Ideally, all records would be captured by the vital signs system. Missing data in the documentation and inconsistent capture of documentation can lead to discrepancies, errors in patient care, and failure to recognize critical trends in patient vital signs, inhibiting the highest quality of care.

The success criteria reviewed during meetings consist of project completion timeline, functional requirements, quality target controls and project deliverables, and keeping stakeholders up to date with any nuances or unforeseen events that could extend the project completion date. The expected outcomes in this practicum project are based on changes in behavior and measurable data. Expected outcomes include: Phoenix VA Healthcare System will shift to structured data entry in the hemodialysis documentation template for nurses with a discrete field to record vital signs. All the entries in that field are captured correctly by the vital signs tracking system and can track and trend vitals signs recorded in the hemodialysis unit. Also, end users continue to use it after the completion of project.

The project will mainly focus on the advancement of the use of information technology.
